New Patient Forms

We are always welcoming new patients to our practice!

At your new patient appointment you will be welcomed by a warm smile from Katie as soon as you walk in the office. After filling out some brief paperwork, Carolann will give you a tour of the office, then take you back to an exam room where she will review your medical history with you.

Next, Carolann will update any necessary x-rays and take photos. As Dr. Jennings Westrick is reviewing your x-rays, you will meet our hygienist, Kristin and she will assess the health of your gum tissue.

Following that, Dr. Jennings Westrick will perform a comprehensive exam, where she will look over any work you've had done in the past and examine the current dental health of your mouth. She will discuss any findings and make recommendations from there. At this time she will also touch base with you about your dental concerns and goals, so we can best assist you!
